The Form is a required document to create a standby guardianship and must be signed by the parent and standby guardian. Any of the forms provided below may be helpful for parents and the standby guardian in creating a family safety plan to protect the children.
Children’s Information Sheet
Lists all of the emergency contact information about your children and important information about their schools, medical conditions, allergies, medical care providers, and after care providers.
School Emergency Contact Information Form
Each school system has a form that parents fill out to authorize someone to pick up the child from school or to contact in the case of an emergency. Montgomery County has one form. Prince George’s public schools use a different one in each school. The parent needs to update this form to ensure that the standby guardian can pick up the child if the parent is detained.

Online Detainee Locator
This is the website that the Standby Guardian and other family members can go to to locate the parent if the parent has been detained. On the Online Detainee Locator you can search either by the Alien Number (A-Number, a nine digit number the parent has been given by ICE) or by their biographical information. To search by biographical information, the standby guardian needs to know the full legal name, date of birth and country of origin of each parent.
Privacy Waiver Authorizing Disclosure to a Third-Party Form
This ICE form 60-001 allows a parent to name a person that they want to receive information about their location if detained. If the parent checks the box in the form that says “Detention Information”, the standby guardian can contact ICE about the location of the parent if they are unable to locate the parent on the Online Detainee Locator.
Revocation Form
The parent can revoke the standby guardianship at any time. If the parent wants to revoke the standby guardianship prior to the standby guardian’s filing of a Petition by Standby Guardian (Judicial Appointment) the parent can either tell or notify the standby guardian in writing that the guardianship is terminated or by any other act that is evidence of a specific intent to revoke the standby guardianship. If the Petition has been filed, the parent must execute a written revocation and file it with the court in which the Petition was filed and also promptly notify the standby guardian of the revocation.
